Taking You Back to Where You Were

I got a question - how come I can occasionally be adding a comment to a pic, and after typing my message and hitting submit, I get "Taking You Back to Where You Were"... without my posting appearing? It's annoying. Am I doing something wrong to cause this? I seldom want to go back and retype the message again. Lately I have started keeping my message text in a paste buffer just in case I have to re-input it.
