Cloud formations are very interesting and unpredictable. They can take the form of virtually anything the mind perceives. I have taken many photographs of clouds resembling strange creatures. I even took one that looks like the hand of God reaching down from the sky. Photoshop was used in many of my pics to increase contrast and colour correct, but that doesn't make them fake. Virtually all photos are enhanced before being published or distributed. I can't tell if the image you are referring to is a fake, but occasionally Mother Nature does use her talent to create some amazing results.
